What to Bring to Your Appointment


Please bring all necessary items to your appointment:

  • Photo ID
  • Insurance card
  • A list of current medications
  • Any previous imaging scans such as X-rays (if available)

Front Desk Check-In

You should arrive 15 minutes early to complete any necessary paperwork.

During Your Visit

Your ÈËÆÞÖгöÊÓƵ's appointment will take place in an exam room.  We understand your ÈËÆÞÖгöÊÓƵ may be anxious. Your provider will take time to talk to you and your ÈËÆÞÖгöÊÓƵ about questions or concerns. Your provider will ask about your ÈËÆÞÖгöÊÓƵ's medical history and symptoms.
