Insurance
ÈËÆÞÖгöÊÓƵ of Utah Home Infusion Pharmacy (UHIP) contracts with most of the major insurance providers in the market. UHIP makes every attempt to work with non-contracted payers on a case by case basis placing patients’ financial responsibility and their interest first and foremost in decision process.

UHIP is a part of the ÈËÆÞÖгöÊÓƵ of Utah ÈËÆÞÖгöÊÓƵ system. We are an alternate site infusion pharmacy primarily providing the following:
Pharmaceuticals
Supplies to patients in home setting, consultation, coordination of services
Our infusion pharmacy is staffed and operated 24 hours a day, 7 days a week, and 365 days a year by professionally trained and licensed nurses, pharmacists, and pharmacy technicians.
Nursing services are provided by qualified Medicare-certified home health care agencies.

How to Prepare for Your Appointment
In order to help us provide you with a quality infusion treatment experience, please review the following tips for how to prepare for your appointment.
- Please drink plenty of water prior to your appointment.
- Abstain from drinking (alcohol and caffeinated drinks) or smoking.
- Bring a current list of all medications you are taking.
- You may bring one visitor to sit with you during treatment.
- Please do not bring ÈËÆÞÖгöÊÓƵren under 14.
Referrals
UHIP referrals may be received directly from the community from home health or hospice, physician’s offices, or emergency departments. Referrals are also received from the ÈËÆÞÖгöÊÓƵ of Utah ÈËÆÞÖгöÊÓƵ system. Freedom of choice is always provided to the patient.
Within the ÈËÆÞÖгöÊÓƵ of Utah ÈËÆÞÖгöÊÓƵ hospitals, UHIP has infusion specialists who assist with care resource/financial resource assessment and coordination of the infusion plan of care. UHIP works with the Huntsman Cancer Institute and has the State of Utah Hemophilia contract. A case management model is followed and services are provided to all the Utah Medicaid hemophilia patients. For more information, please see our referring providers page.
UHIP is licensed as a pharmacy by the Utah State Board of Pharmacy and accredited by The Joint Commission meeting home care standards. UHIP provides pharmacy services throughout the state of Utah.
