Evaluation Process for Admission

To inquire about an admission to the Young Adult Comprehensive Assessment and Treatment (YA CAT) program for you or a loved one, please contact our admissions coordinator and outreach specialist at 801-581-6987. Our admissions team will speak with the referring individual to gather as much information as possible, as well as providing information about our program.

Our admissions coordinator will then present the information to the multidisciplinary CAT leadership team to determine if the individual is an appropriate fit for the program. Once our CAT leadership team approves an individual for our program, we contact the referring individual to begin the application process. During this time in the process, we will set up a call between the prospective individual and our clinical director to further discuss the program and answer any additional questions that the individual may have before coming to the program.  

Once the application process is complete, the YA CAT administrative assistant will reach out to schedule an admission date and connect them to the HMHI business office to discuss and finalize the financial aspects of the program.

How to Prepare for Admission

When available, previous medical records such as past assessment results, medication trials, and treatment history helps guide our treatment team as they assess the individual. It's highly recommended that we receive these records either prior to the admission or bring them on the day of admission. This gives our treatment team more time to review prior evaluations efficiently instead of spending time obtaining records. 

Due to HIPPA laws, other facilities cannot send us records they did not generate. This means that the last treatment facility can only send their own records even if they have copies of an individual's records from elsewhere.

What to Expect Day of Admission

On the day of admission, the individual will arrive to the facility at a previously agreed upon time. Upon arrival, the individual will meet with the treatment team, receive a tour of the facility, and be orientated to the program.

The involvement of family during an individual鈥檚 stay in the program can be helpful on multiple fronts. We will talk with a client's parents or loved ones to get their perspective on the individual. These intake meetings may occur with the family in-person or on the phone. The intake meeting serves as an opportunity for parents and/or guardians to provide the treatment team with recent presenting issues, a thorough developmental history, and their goals for the CAT Program. 

Program Cost & Financial Questions

We intentionally keep the treatment team focused on providing the best clinical care for our clients. Our admission coordinator will connect you to the business office during the admission process.

The building for our program embodies more of a home-like feel rather than that of a hospital or institution. We take great pride in providing an atmosphere that is both safe and comfortable for individuals during their time in the program. Our spaces allow individuals to either enjoy each other鈥檚 company or enjoy their alone time. All individuals in the YA CAT program will have their own bedroom and bathroom. We believe it's important for individuals to have a space that is solely theirs.
